Make sure that it is in the <head> area of the header.thtml and make sure it looks like this:
<style type="text/css">
@import url("/mantis/css/default.css"
</style>
You may need to change "/mantis/css/defualt.css" depending on where in your web path you installed mantis.

I have mantis intergrated on my site, and all I did was copy the code from the mantis css file and paste it into my theme file css, rename the mantis css file therefore it uses the code in the theme css!
Worked for me anyway, I did it like this because I run multiple themes and needed mantis to intergate into each one!
