PHP Form Login Required

Hello,

I recently installed PHP Form Generator on my GL site and put up a simple form (http://www.jeffandcrystal.com/submitaddress) that collects address information. I noticed that the form requires visitors to login which I'd like to disable, but cannot figure out how to do.

Can someone let me know how to let visitors use a form without having to login?

Any information that you may be able to provide would be greatly appreciated, thanks!

Not an expert but at first glance looks like there is a htaccess file (or some passwd) in the submitaddress directory. Just delete it. Should work.
