I know I'll figure this for myself eventually, but the quicker I get it the better because of a quick-breaking story, so I'm asking for help.

I need to create a very simple email form for a news article I'm entering, where the user enters the subject, from-email, and body in text areas, but I already set the destination.

(Basically I'm inviting visitors to join me in sending email to a public entity doing something that I consider mucky.)

I know, I know, simple stuff to code and test, and I'm starting right now, but does anybody currently online have a quick-and-dirty I could just hack?
