1) I seem to recall a theme a few years ago that had the login and password in the header.thtml. Does anyone recall this and is it available?

As for #2: You can use PHP in header.thtml, so nothing's stopping you there ...

1) I think that was with Geeklog in the 1.2X series. Easiest possible way to do this is modify your template to have a hardcoded login form
