Problems after site upgrade

I just upgraded from 1.3.11sr2 to the latest 1.4 code. Overall, the upgrade went smoothly. I modified my theme, copied over the new dirs and did the upgrade process. All in all, things look good. To see it, here's the URL:

However, there are still some problems:
fsckEditor
It does not appear that the advanced editor is loading completely. The tabs load but clicking them does nothing. Above each text box are two broken image icons and there are no WYSIWYG icons that I would expect. I am assuming this is related to some theme things that did not get migrated over but I don't know what. Anyone have any suggestions?
Commenting
When a user clicks to leave a comment, it gives two text boxes. What is the second one for?
Remote Authentication
This does not appear to be working. I have the two entries correct in config.php and the dropdown menu of services appears, but logging in with a valid blogger ID returns a message that the username or password was incorrect (as though the user tried to incorrectly log into the site.) The log file simply records a failed login, nothing service-specific.
Links Block
I had a links block in 1.3.11 and the movement of the links to a plugin broke it. How can I restore it?

OK, I fixed the links issue.
As for the other three, I decided to try with the default Professional themes from the 1.4.0 package. All three of the other issues still occur with the default theme.
Something weird is going on here. Any ideas on how to track this down and fix it?

There should be only one field. My guess is that you're missing some JavaScript code that would hide the second field.
Did you add {advanced_editor} to your theme's header.thtml?
