Site survey with multiple polls

Hello friends,
I would like to create a page with a sort of "site survey", with multiple questions and answers, like this:

Age:
- 15-20
- 21-25
- 26-30
....

Where did you find us?
- Search Engine
- Friends
- Magazines
....

How often you visit us?
- Every day
- 1 time / week
- 1 time / month
.....

Well, I think the simplest way to create this survay is a "multiple poll" hack.... 6 or 7 polls in the same page, or something like this.

Is there a hack/plug-in/suggestion to do this?

Well, I think the best way to solve my question is to create a "special poll" hack....
It's very simple (but I'm not able to do): I could create 10 or 15 polls, one for each question, then I create a web page with the link of the first poll. When I click "Submit" I have to go in the second poll, and so on, until the last poll.
The only questions are:

- How to create a "specialpoll.php" file with this modification?
- How to hide the group of 10/15 polls in the geeklog's poll page, and to show only the "normal" polls?

I hope someone can help me.

You're effectively asking for someone to tell you how to write a PHP script. I'm afraid that's not going to happen ...

Maybe Blaine's glQuiz plugin would work for you.
