Installed the downloads plugin.
Files folder is placed outside webroot.
On the category admin page, I can set permissions, but cannot select groups.
No drop down menu.
Is this intended? How to enable this?

There should be a drop down list for groups.

How come its not there for me?
I can manipulate group id in database, and permissions is then correct.
But in category edit instead of the drop-down, only the set group is displayed as text.

Argh...
When browsing the plugin section in the download-area here, an old version of the plugin is displayed before the newest version....
Found the newest version, or at least V 1.0.4, and then the problem was fixed.
